The Keerthika, a student at a local government school, was reportedly riding on a motorcycle with her father, Durga Prasad, and her 10-year-old brother, Sai, returning from school. The TSRTC electric bus allegedly struck the motorcycle, causing all three occupants to fall onto the road. Keerthika sustained a severe head injury and was pronounced dead at the scene. Her brother, Sai, suffered fractures, while their father sustained minor injuries.

Initial police accounts suggest the bus was being driven in a "rash and negligent manner."
